The Sites are intended to facilitate the provision of the Services to registered users.
The "Services" may include (i) providing individuals with information on health care and wellness; (ii) pharmacy services; (iii) technology-oriented tools; (iv) development of health-care records; (v) administrative support for scheduling and payment; and (vi) telecommunications support for accessing health-care providers provided by affiliated professional entities.
Availability
Certain of our Services are currently only available to individuals located in certain states.
Eligibility
In order to qualify to use the Services, you must be age 18 or over, located in a state where we operate, and agree to be legally bound by and comply with these Terms of Use. Satisfying these does not guarantee you will receive Services through Kiln Holdings LLC. Kiln Holdings LLC and certain affiliated professional entities reserve the right to change or include new requirements at their sole discretion without prior notice.
Requirements for Use
You must have compatible computing and/or mobile devices, access to the Internet, and certain necessary software in order to use the Sites. Fees and charges may apply to your use of mobile services and the Internet.
Special Consent to Telehealth Services
Telemedicine involves the delivery of health-care services using electronic communications between a health-care provider and a patient who are not in the same physical location. It may be used for diagnosis, treatment, follow-up, and related patient education. The electronic systems used in the Services incorporate network and software security protocols to protect the privacy and security of health information. Your Relationship With Kiln Holdings LLC
Kiln Holdings LLC does not provide any medical services. Rather, Kiln Holdings LLC provides a technology platform for you to access a health-care provider employed or contracted with affiliated professional entities. By coordinating and consulting with an affiliated provider through the Services, you are not entering into a provider-patient relationship with Kiln Holdings LLC. You agree that we will not be liable for any loss, injury, or claim arising from your failure to read communications from us or providers, or from your failure to comply with treatment recommendations.
Communications by Text Message and Email
By providing your mobile number and opting in, you agree to receive account, online-visit, support, transactional, and, where separately opted in, promotional text messages from Curaida. Message frequency varies. Message and data rates may apply. Reply STOP to opt out and HELP for help.
Consent to receive text messages is not a condition of purchase. You represent that you are the owner or authorized user of the mobile number you provide and that you are authorized to approve any applicable message or data charges.
Pharmacy Services
If you receive a prescription as a result of the Services, you may select Kiln Holdings LLC or one of our partner pharmacies to ship your prescription. You consent to our sending the pharmacy network all information necessary to provide pharmacy services.
Payment
You agree to pay all fees due for services requested. By entering your payment information and submitting your request, you authorize us, our affiliates, or our third-party payment processors to charge the amount due. You are responsible for all fees due to receive health-care and pharmacy services, including any fees charged by the health-care organization(s) or provider(s). Your payments to Kiln Holdings LLC may include fees charged by health-care organization(s) or provider(s) collected on their behalf.
Trademarks and Links to Third-Party Sites
Certain names, logos, and other materials displayed on the Sites may constitute trademarks or service marks of Kiln Holdings LLC or its affiliates. You are not authorized to use them without our express written permission. The Sites may contain hyperlinks to other websites operated by third parties. We do not control those Linked Sites and are not responsible for their content.
Termination
These Terms of Use remain in effect as long as you continue to access or use the Sites or Services. Kiln Holdings LLC may terminate or suspend any rights granted by these Terms and your access to the Sites or Services at any time, with or without notice.
